DFHA Facility Participation and Media Waiver Agreements

Delaware Field Hockey Association (DFHA) Media Release Waiver

By participating in Delaware Field Hockey Association (DFHA) events, programs, practices, or activities, participants and/or their parents or legal guardians acknowledge and agree to the following terms regarding media usage.

Media Consent

DFHA reserves the right to capture photographs, video recordings, and other forms of media during its events and activities. These materials may be used by DFHA for promotional, educational, and marketing purposes, including but not limited to:

Social media platforms
Websites and email communications
Printed materials, flyers, and brochures
Local news coverage and advertising

These images and recordings may feature participants individually or in group settings and may be used without restriction for future DFHA-related promotions.

Parent/Guardian Acknowledgement for Minors

For participants under the age of 18, parental or legal guardian consent is required. By agreeing to this waiver, the parent or guardian grants permission to DFHA to use photographs, videos, or other media of their child in the manners described above, without compensation or further approval.

If a parent or guardian does not wish for their child to be photographed or recorded, written notice must be submitted to DFHA prior to the child’s participation in the activity.

Opt-Out Process

Parents or guardians who do not consent to media use must:

Submit a written opt-out request via email to [contact@delawarefieldhockey.com]
Provide the participant’s full name, program name, and dates of participation
Acknowledge that DFHA will make reasonable efforts to exclude the participant from media, but cannot guarantee full exclusion from wide-angle or group photos

Agreement

By participating in DFHA programs or by checking the boxes on the registration forms, participants and/or their parents/guardians indicate they have read, understand, and agree to this media release policy.

University of Delaware Rullo Stadium Facility Participation Waiver

Delaware Field Hockey Association LLC (DFHA) is a nonprofit organization that offers recreational adult field hockey leagues, practices, tournaments, and social events primarily hosted at the University of Delaware’s Fred P. Rullo Stadium, located at 645 S College Ave, Newark, DE 19713 (“Rullo Stadium”). DFHA rents the Rullo Stadium for these activities, and this agreement applies to all events and facilities provided by DFHA.

For the purposes of this waiver:

“DFHA Parties” refers to DFHA’s Board of Directors, members, representatives, officers, employees, volunteers, owners, and all successors, heirs, and assigns.
“Rullo Stadium Facilities” refers to the field, stadium, and all equipment, fixtures, or property used at the location.

Waiver of Liability, Release, Assumption of Risk, and Indemnification Agreement

1. Acknowledgment of Risk and Voluntary Participation

By participating in DFHA activities at Rullo Stadium, I acknowledge that field hockey, like all physical activities, carries inherent risks, including but not limited to bodily injury, property damage, and death. I voluntarily assume all such risks associated with my participation in DFHA’s activities.

2. Release and Waiver of Liability

In consideration of being allowed to participate in DFHA activities, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby fully and forever waive, release, discharge, and hold harmless DFHA and all DFHA Parties from any and all claims, demands, actions, or causes of action related to any injury, property damage, or death arising from or connected to my use of Rullo Stadium Facilities or participation in DFHA activities.
This release applies regardless of whether the injury or damage is caused in whole or in part by the negligence, recklessness, or fault of DFHA Parties.

3. Covenant Not to Sue and Indemnification

I agree that I will not sue or bring any claim against DFHA or any DFHA Parties in connection with my participation. Should I, or anyone on my behalf, pursue such a claim, I agree to indemnify and hold DFHA harmless from all costs, including legal fees and any awarded damages.
If DFHA is named in any claim where I seek damages from another party related to my participation, I agree that any damages awarded will be reduced by the proportional share of liability attributed to DFHA.

4. Governing Law

This agreement shall be governed by the laws of the State of Delaware. If any part of this agreement is found invalid or unenforceable, the remaining provisions shall remain in full force and effect.

5. Acknowledgment of Understanding

Agreement

By participating in DFHA programs or by checking this box on the registration forms, participants and/or their parents/guardians indicate they have read, understand, and agree to this waiver, acknowledging that:

I have carefully read and fully understand this agreement.
I am voluntarily waiving substantial legal rights, including the right to sue.
I am aware of the inherent risks involved in field hockey and voluntarily accept them.
I have not relied on any statements made by DFHA representatives outside of this written agreement.

DE Turf Facility Participation Waiver

Delaware Field Hockey Association LLC (DFHA) is a nonprofit organization that offers recreational adult field hockey leagues, practices, tournaments, and social events primarily hosted at the DE Turf Sports Complex, located at 4000 Bay Rd, Frederica, DE 19946 (“DE Turf”). DFHA rents the DE Turf for these activities, and this agreement applies to all events and facilities provided by DFHA.

For the purposes of this waiver:

“DFHA Parties” refers to DFHA’s Board of Directors, members, representatives, officers, employees, volunteers, owners, and all successors, heirs, and assigns.
“DE Turf Facilities” refers to the field, stadium, and all equipment, fixtures, or property used at the location.
